Kitten/Cat List Entry Form

You must be a fully paid-up and approved member of the club to use this form. Please complete a separate form for each litter of kittens or each cat. All kittens entered must be a minimum of 4 weeks of age. Queries regarding the Lists should be made to Adrian Miller by email or to Dee Marsland-Round by telephone (01932 886728)

Membership No.	All Membership Numbers are 4 digits (if you have a 3 digit number prefix it with "1")
Your Surname
Your Post Code

Cat/Kittens Date of Birth:	(in the format dd/mm/yy)

Please group all kittens of the same sex and colour together on one line and alter the quantity accordingly. If you don't know the GCCF Breed Number then either refer to your registration slip or our Website Page - Breed Numbers
